Emily’s father’s health was beginning to deteriorate, and her mother was recently diagnosed with dementia.

While Emily lived near her parents, her brother lived an hour away. And with their busy work schedules and not always being able to be at their parents’ home, the siblings were unsure how to get their parents the care they needed.

  2. Worked with the care team to arrange in-home care, researched assisted living and local memory care centers, arranged transportation for appointments, and coordinated Meals on Wheels services
  3. Spoke with the father about managing his chronic condition and medications, and facilitated evaluation with a counselor to address his depression related to his wife’s illness
