The confidential online Health Risk Assessment (HRA) questionnaire can help you determine your risk for a variety of diseases and conditions and provide informational guidance for reducing your risk.

Here’s how the HRA works:

It takes minutes. The questionnaire asks about your medical history, medications, health habits and other basic health information.

Fast results. Almost immediately upon completing the HRA, your screen will display a complete profile of your general risks associated with up to 13 diseases and conditions: cancer (breast, cervical, melanoma, uterine, ovarian, prostate, lung, colon and stomach); heart disease, stroke, diabetes and osteoporosis.

Resources you need. Your profile also includes health information about how to reduce your risks developed by the renown Mayo Clinic.

Enhances wellness coaching. The HRA can be a helpful tool for one-on-one guidance from a personal wellness coach. Your coach can help you set workable action steps to reduce your risks such as losing weight and help boost your overall health and wellbeing.

Completely confidential. The HRA information is secure and cannot be accessed by your employer or anyone else.

If you wish, you can share your results with your physician who can work with you to help you reach personal health goals and reduce your risks.

And if you are participating in the personal wellness coaching program, you can also share your results with your personal wellness coach. Should you choose to do so, our coaches are specially trained to maintain strict confidentiality in compliance with governmental privacy regulations about any health information you provide. No one else sees your answers or the results to the questionnaire.
